All our services, including home hospice care, day hospice care and Star PALS are provided to patients at no charge, regardless of age, religion, ethnicity, nationality and financial status.
Our services are available to all patients who are referred to HCA by their primary physician from the hospital or a private clinic. They will need to complete and submit the patient referral form found below.
All of HCA’s services are provided at no charge to patients and their families. To support our funding arrangements, we will seek your consent for HCA to retrieve your applicable government subsidy rate under the National Means Testing System. If the information is not available, our Medical Social Worker will guide you through the process and assist with completing the required form.
Upon admission, we require the following from patients, their spouses and household members living at the same residence as the patient, as reflected on the NRIC/FIN/Special Pass/Foreign Passports.
