Home Hospice

We are Singapore’s largest home hospice care provider and we support patients with life-limiting illnesses and their families in the comfort of their homes. Our dedicated multidisciplinary healthcare teams are on call round-the-clock, seven days a week, making more than 38,000 home visits islandwide every year. This service is provided at no charge.

What We Do

Our multidisciplinary team comprises doctors, nurses, medical social workers, patient care administrators, as well as trained volunteers, who offer support and care based on their areas of expertise and the particular needs of patients and their families. Also available are the services of a spiritual care counsellor, art therapists and in-house pharmacists. In addition, we offer a 24-hour patient care helpline that is available to patients and caregivers, for round-the-clock assistance.

Our Multidisciplinary Team

Medical Professionals
We work with primary physicians in clinics and hospitals to determine the necessary care plan best suited for the patient. After patients are admitted into our home hospice service, a primary nurse is assigned to provide care, manage pain and symptoms, as well as to guide families on caring for patients at home. A doctor may also review patients’ medication and issue prescriptions for pain or symptom control.
Counsellors and Medical Social Workers
Our counsellors and medical social workers provide social, emotional and spiritual support through individual and family counselling, to help patients and their families cope with the diagnosis, grief and loss.
Volunteers
Our non-medical volunteers provide essential social and emotional support by befriending patients and helping with daily activities like light housekeeping and grocery shopping.
