Medi Minder Coordinator

What You Will Do

The incumbent will be responsible for coordinating the Medi-Minder (MM) assignments for the purpose of respite care. He/ She will also participate in all relevant MM activities and follow up care as assigned by the doctor or nurses.

The scope of work will generally encompass the following duties:

  • Arranges Medi-Minder (MM) assignments for Star PALS home care patients.
  • Briefs and debriefs MM both before and after assignment and coordinates MM Surveys for Caregivers and MMs after each assignment and provide support to MMs.
  • Functions fully as a competent MM after completing relevant training and BCLS (include infant and child module) competency requirement.
  • Takes over MM assignment when no MM is available but family is in need.
  • Conducts recruiting, selection and interviewing MM and Oasis Minder (OM) candidates together with Oasis Minder Coordinator (OMC).
  • Together with OMC, coordinates MM’s training and continuous education and support meetings as part of these volunteer staff engagement.
  • Together with OMC, coordinates Star PALS events that involve MM/OM.
  • Participates in implementing the comprehensive MM/OM training package, using the ‘competency checklist’ for the purpose of accreditation.
  • Follows up and address MMs’ skills and concerns during the 1st assignment to a patient’s home when an assignment was arranged (MMC with registered nurse qualification).
  • Follows home care nurses on home visits to new patients to facilitate rapport building and promote MM services.
  • Refers the MM to the psychosocial team if a MM needs formal counselling support.
  • Reviews MM/OM service or training content including competency checklist every two years, in consultation with Star PALS team and OMC.
  • Attends weekly Star PALS meetings to be updated on patients’ latest clinical condition and to anticipate concerns that may arise during MM assignments.
  • Reports any newly developed/unusual medical problem or care issue during MM assignments to the primary nurse and/or palliative physician.
  • Prepares monthly MM statistics report for MM assignments in the home setting.
  • Prepares quarterly MM service report for home based MM assignments.

Who You Are

  • Diploma in Nursing
  • Preferably 2 years of relevant experience, with some paediatric experience
  • Good communication and interpersonal skills
  • Good multi-tasking and time management skills
  • Able to work efficiently in MS Excel, Word and PowerPoint
