HCA Home Hospice

As the largest hospice home care provider in Singapore, HCA Hospice Care has five dedicated multidisciplinary health care teams who make over 38,000 home visits island-wide every year. They are on call round-the-clock, seven days a week. Besides serving from our central headquarters at 12 Jalan Tan Tock Seng, we have four satellite centres spread across Woodlands, Hougang, Bedok and Jurong to be more accessible to patients.

Each team comprises a doctor, four to five nurses, one medical social worker, one social work assistant and trained volunteers who offer support and care based on their areas of expertise and the particular needs of patients and their families.

HCA’s hospice home care service includes:

  • Providing nursing and medical care to manage patients’ pain and symptoms.
  • Providing medical prescriptions.
  • Coaching families on how to care for their loved one at home.
  • Assisting patients and their families with the emotional and social aspects of coping with death, grief and loss.

HCA Home Hospice in a nutshell

When a patient is admitted into our home care service, a primary nurse is assigned to him or her for regular home visits. This nurse will provide care, manage pain and symptoms, as well as coach families on how to care for the patient. A doctor may also review the patient’s medication needs and issue prescriptions for pain and symptom control.

For patients and families who need emotional and spiritual support, the primary nurse will introduce a medical social worker to assess their needs and help them resolve issues.

In addition, non-medical volunteers work directly with patients and their families, helping with daily activities of living such as housekeeping, buying groceries and providing social, emotional and spiritual support. Also available to our needy patients is a loan service for equipment such as wheelchairs and hospital beds.

HCA Hospice Care also provides an after-office-hours patient care help line to patients and caregivers.
