Hospice Home Care Service
As the largest hospice home care provider in Singapore, HCA has five dedicated multidisciplinary health care teams who make over 30,000 home visits island-wide every year. They are on call round-the-clock, seven days a week.
Each team comprises a doctor, four to five nurses, one medical social worker, one social work assistant and trained volunteers who offer support and care based on their areas of expertise and the particular needs of patients and their families. The teams each take care of patients at designated geographical areas throughout Singapore.
HCA's hospice home care service includes:
- Providing nursing and medical care to manage patients' pain and symptoms.
- Providing medical prescriptions.
- Coaching families on how to care for their loved one at home.
- Assisting patients and their families with the emotional and social aspects of coping with death, grief and loss.
In addition, non-medical volunteers work directly with patients and their families, helping with daily activities of living. Also available to our needy patients is a loan service for equipment such as, wheelchairs and hospital beds.
When a patient is admitted into our home care service, a primary nurse is assigned to make home visits to patient regularly. This nurse will provide nursing care to manage the patient's pain and symptoms, coach families on how to care for the patient, and when appropriate, to request the doctor to review the patient's medication needs. The doctor will provide prescriptions for medication to relieve the patient of his pain and to control his symptoms.

For patients and families who need emotional and spiritual support, the primary nurse will introduce a medical social worker to assess their needs and help them resolve issues.
In addition, non-medical volunteers will work directly with patients and their families, helping them with daily activities of living such as assisting in house cleaning, buying groceries and providing social, emotional and spiritual support.
HCA also provides an after-office-hours patient care help line to patients and caregivers.
