Admission
Our hospice home care and day care centre services are available to all patients who are referred to HCA by their doctors. The doctor can either be the patient's consultant at a hospital or a general practitioner from a private clinic. All that the doctor needs to do is to complete our patient referral form [56kb pdf].
A copy of the form can also be mailed to anyone who calls us at our main telephone line 6251 2561.
